Quality of Life Interview

For this survey to be completed, the following equipment is needed,

The survey sheet
A pen/pencil
10 people to question

10 members of the public are needed for you to ask them to answer 6 questions. The public can be fount near shops, on the streets or if no one was round, say on a rainy day, it would be relevant if you polity knock on a door and then ask the questions.

The 6 questions are: -

Your house
Number/type/quality of shops in the area.
Friendliness of the area
Your personal security and safety
The appearance and tidiness of the area
There age

Read out the questions to the person then the scores. Ask him/her to tell you the score of the questions. The scores are 2 for extreme, 1 for satisfied and 0 for dissatisfied, once they have told you then you can write down the score on the survey sheet.
