In my study I put the following extra fields, patient Number, Telephone Number, Patient Status, Number of Items in Stock and Cost of Replacement. The reason I put in patient number was because I will use this as a key entity to identify each patient. I put in telephone number because I would use this number to contact the patient if they are at home. I used Patient Status so that I would know if the patient was in or out of the hospital premises. I added Number of items in stock because this would be used to fill in the report weekly. Finally I added cost of Replacement because then I would know how much each item would cost to be replaced.
