Introduction

        I have been employed to produce a computer application to check and keep record of loans to patients of equipment such as wheel chairs and crutches. Records must consist of the following data, what has been borrowed, to whom it is been loaned to, when it was given for loan and if any equipment is not available because it is damaged and waiting to be repaired.

In my study I put the following extra fields, patient Number, Telephone Number, Patient Status, Number of Items in Stock and Cost of Replacement. The reason I put in patient number was because I will use this as a key entity to identify each patient. I put in telephone number because I would use this number to contact the patient if they are at home. I used Patient Status so that I would know if the patient was in or out of the hospital premises. I added Number of items in stock because this would be used to fill in the report weekly. ...
