Assessment template

Please choose one case study and then use this template in order to produce a structured consideration of the health care needs and service provision for one client or group. You will obviously have to make reasonable assumptions about the individuals concerned. Simply tell us you have made an assumption and explain why.

In order to pass the assignment you must: meet ALL of the marking criteria; use this template and use a scenario assigned to your group.  Please make sure you address all criteria by completing all sections of the template. Make sure you have the right template!

If you require clarification with regard to any aspect of this assignment please obtain it prior to submission of your script. You are advised to arrange to download the template in the first 2 weeks of the module. This will ensure you have plenty of time to make arrangements to use the technology.  It is your responsibility to ensure you can do this. . Case Study – two

George is a recently widowed 70 year old gentleman who cares for his 50 year old learning disabilities son Paul. Paul is loving and affectionate but demands care and attention. He is unable to live independently and is dependant on George for assistance with his daily living activities.

George loves his son and enjoys his company. But George misses his wife and feels desperate in his struggle to manage the care of his son on his own. There are no other relatives, although his elderly neighbours will go to the local shop for George if he needs anything. But he doesn’t like to bother them and money is a worry.

When his wife was alive, he was able to meet his old work colleagues for an hour, once a week, at the local working men’s club. That’s all finished now, for he promised his dying wife that he would not see Paul placed into care while he was alive, to look after him. George is tired, losing weight and is finding it increasingly difficult to sleep. He is worried now that he is on his own, about how much longer he will able be able to cope.
